Pain Management in the 21st Century
View the Podcast here. Chronic pain affects nearly 90 million Americans. It usually begins with a sports injury, car accident or health condition like migraines, diabetes, arthritis and cancer.  Chronic pain is different from the acute pain of stubbing your toe and often feels like burning, shooting, or shocking sensations.  The good news is that today’s pain specialists have sophisticated new treatments — from medications to advanced technologies — to provide chronic pain relief.    Chronic pain can be classified as nociceptive or neuropathic pain.  In some cases (nociceptive pain) the body’s ner...

Impact of the COVID-19 pandemic on people living with migraine: Results of the MiCOAS qualitative study
CONCLUSION: The global COVID-19 pandemic has resulted in both negative and positive impacts for people living with migraine. Lessons to be considered when moving into a post-pandemic world include benefits of and satisfaction with telehealth and the benefits and importance of healthy lifestyle habits and flexibility such as improved sleep, reduced stress, and fewer social expectations.PMID:35294046 | DOI:10.1111/head.14274

Migraine pathophysiology: lessons from mouse models and human genetics
Publication date: January 2015 Source:The Lancet Neurology, Volume 14, Issue 1 Author(s): Michel D Ferrari , Roselin R Klever , Gisela M Terwindt , Cenk Ayata , Arn M J M van den Maagdenberg Summary Migraine is a common, disabling, and undertreated episodic brain disorder that is more common in women than in men. Unbiased genome-wide association studies have identified 13 migraine-associated variants pointing at genes that cluster in pathways for glutamatergic neurotransmission, synaptic function, pain sensing, metalloproteinases, and the vasculature. The individual pathogenetic contribution of each gene variant is diffi...

A migraine management training program for primary care providers: An overview of a survey and pilot study findings, lessons learned, and considerations for further research
ConclusionsThough PCPs are familiar with many aspects of migraine care, there is a need and opportunity for improvement. The three live sessions were poorly attended and the email consultative service was rarely used. We provide an in depth discussion of targeted areas for educational intervention, of the challenges in developing a migraine educational program for PCPs, and areas for future study.

Employee and Employer Benefits From a Migraine Management Program: Disease Outcomes and Cost Analysis.
CONCLUSION: The employer-sponsored disease management program provided a better understanding of migraine, promoted methods and approaches to improve management by combining medical and lifestyle options leading to significant improvements in migraine symptoms that sustained beyond the intervention, supporting prolonged effectiveness of such programs. The program also provided a high ROI to the employer, supporting that the systematic inclusion of such programs into corporate well-being initiatives can be of significant benefit not only to the impacted individuals but to the employers as well. Differences in treatment response between migraine with aura and migraine without aura: lessons from clinical practice and RCTs
AbstractMigraine is a major public health problem afflicting approximately 10% of the general population and is a leading cause of disability worldwide, yet our understanding of the basis mechanisms of migraine remains incomplete. About a third of migraine patients have attacks with aura, consisting of transient neurological symptoms that precede or accompany headache, or occur without headache. For patients, aura symptoms are alarming and may be transiently disabling. For clinicians and scientists, aura represents an intriguing neurophysiological event that may provide important insight into basic mechanisms of migraine. ...

Effectiveness of the traditional Japanese Kampo medicine Yokukansan for chronic migraine: A case report
Rationale: The traditional Japanese Kampo medicine Yokukansan (TSUMURA Yokukansan extract granules) was originally used to treat neurosis, insomnia, night crying, and irritability and/or agitation in infants and recently it has also been used for neuropsychiatric symptoms in Alzheimer's disease or other dementia in Japan. Furthermore, several recent studies have reported the efficacy of Kampo medicines for various types of headache. Here, we report a case of severe chronic migraine refractory to prophylactic therapy using various western medicines and Japanese Kampo medicines that had resulted in a leave of absence from ...

Short-term effectiveness of an online behavioral training in migraine self-management: A randomized controlled trial.
Abstract Behavioral training (BT) is recommended as a supplementary preventive treatment for migraine. Online interventions have been successful in promoting health behavior change, the evidence for online BT in migraine is limited, however. This randomized controlled trial aimed to determine the post-treatment effectiveness of online BT (n = 195) compared to a waitlist control group (WLC; n = 173) on migraine attack frequency (primary outcome), headache self-efficacy and locus of control (secondary outcomes). BT aims to counteract attacks in the prodromal stage through early detection of prodromal features an...

Pacemaker implantation for treating migraine-like headache secondary to cardiac arrhythmia: A case report
Rationale: Co-occurrence of headache and arrhythmia is not rare. However, their causal relationship remains unclear. Here, we described a case of migraine-like headache relieving with pacemaker implantation. Our case study indicates that arrhythmia is causal for migraine-like headache, which, to our knowledge, has never been reported. Patient concerns: A 63-year-old woman patient suffered from paroxysmal headache with a visual aura presenting like migraine for 2 years. No ophthalmic or neurological disorder was found, but cardiac examination detected bradycardia, which was confirmed by 24-hour dynamic electrocardiogra...

Review Migraine pathophysiology: lessons from mouse models and human genetics
Migraine is a common, disabling, and undertreated episodic brain disorder that is more common in women than in men. Unbiased genome-wide association studies have identified 13 migraine-associated variants pointing at genes that cluster in pathways for glutamatergic neurotransmission, synaptic function, pain sensing, metalloproteinases, and the vasculature. The individual pathogenetic contribution of each gene variant is difficult to assess because of small effect sizes and complex interactions. Six genes with large effect sizes were identified in patients with rare monogenic migraine syndromes, in which hemiplegic migraine...
